Please fill out the order form below and submit order. Then send payment (Cashiers check, Money order, or Personal Check) to PO Box 51 Kahului, Hawaii 96733. Once we have received your processed payment your merchandise will be sent out.

Print out a copy of this form send it with a check or US money order to:  Kahiwa's Kreations PO Box 51, Kahului, Hi, 96733. Orders will not be processed until full payment has been received.
